Specialised Support Services can help clients to achieve their goals.
The support worker will work with clients to develop and achieve goals set out in their health plan.
This can include support for clients to get to appointments, researching on the clients behalf, organising referrals, some advocacy.
We also have limited translation support for Chinese, Croatian and Spanish speaking people

The Access & Support Program works with people in the Hume and Moreland region with unmet health issues who are eligible for CHSP Services. These include but are not limited to:
• Home-help
• Meals on wheels
• Shopping assistance
• Community health services
• Personal care
• Aged Care Assessment Service

Assistive Technology Australia (formerly known as Independent Living Centre NSW)
Assistive Technology Australia provides information and advice to people with disabilities, older people, carers, service providers, allied health professionals and the general community on assistive technology solutions for independent living.
We have a free information service to anyone needing advice on Assistive Technology solutions, home modifications and design ideas. We have built a full sized one bedroom apartment, with home modifications in situ, into the new display at Blacktown. Built to the Livable Housing Design Guidelines, showcasing assistive technology devices, involving high tech as well as simple and good design.
We run group tours, community talks and workshops to spread awareness about Assistive Technology and its role in aiding independent living.
EACH Access & Support
The Access and Support (Specialised Support Services) provides short term, individual support for people who need assistance to access services so they can stay at home. EACH specialised in supporting people with dementia and memory loss who are socially isolated

Monash Health Community, Community Support Options
A&S services play a key role in supporting access to services and improved service responses for people who may be disadvantaged due to their diversity characteristics. A&S services often support people at various stages throughout the client pathway.
People accessing A&S services must be:
• seeking aged care services such as CHSP
• have a diversity characteristic and face barriers to accessing services or staying in services because they:
– are unsure about how to communicate their need for assistance
– have little or no knowledge about aged care services
– have little or no experience in using aged care services
– are not aware of their eligibility for aged care services or lack confidence to find out if or how aged care services could potentially help them.
