Apply for an assessment online

Before you can access government-subsidised aged care services for the first time, you need to be assessed.

You can complete the application for yourself or on behalf of a family member or friend. If you’d like to set up a registered registered supporter, you can also do that here. We recommend completing it together, in the same place at the same time. This makes the signing and consent process easier.

You can use this form to apply for all types of care and support including help at home, short-term care, care in an aged care home, and end-of-life care.

  • Part A checks your age and needs requirements.
  • Part B confirms who the assessment organisation should contact to arrange the assessment.
  • Part C is optional and lets you request to register a supporter. The easiest way to set this up as part of this form is for both of you to complete this together in the same room at the same time. Learn more about what registering a supporter means.

If you've already had an aged care assessment, are registered with My Aged Care, or need immediate care, please do not use this form and call My Aged Care on 1800 200 422 instead.

What to expect in this form

Part A: Describe your situation

Here you will be asked for some information about yourself. We'll check you meet the age and needs requirements and ask some questions about your situation.

This step will take about 10 minutes.

Part B: Share your details

Next, we will ask you to provide some personal and contact details and tell us who the assessment organisation should contact to confirm your needs and arrange the assessment. You will need your Medicare card for this step. If you're applying on behalf of someone else, you'll need to provide their information.

This step will take about 5 minutes.

Part C: Register a supporter relationship (optional)

This final step is optional. In this step, you can request to register a supporter relationship. A registered supporter is a person who can help you make and communicate your own decisions, and access and update your information.

Make sure you understand the role of a registered supporter and how the supporter relationship works before you start.

This step will take about 5 minutes.

After submitting your form 

After you submit your form, the assessment organisation will contact you to let you know the outcome of your application and what to do next.

For requests to register a supporter relationship, the next steps depend on your application. If more information is required, you or your prospective supporter may need to contact My Aged Care. You can check the status of a request to register a supporter through your My Aged Care Online Account.
