Gippsland Multicultural Services CALD Access and Support Program
GMS CALD Access and Support Program assists people from CALD communities who are over 65yrs of age and are experiencing barriers due to their diversity in accessing aged care services. Access and support worker is able to inform the clients on aged care services available and support them in accessing those services according to their needs.

Short term support with housing, social & health issues. People 50+ within Port Phillip or Stonnington with complex care issues who live in Insecure housing eg;
pension-level SRS
rooming houses
community housing
private motels
transitional housing
Community Connections Program (CCP):
Aged 50+ experiencing or at risk of homelessness or living in insecure or low-cost housing:
multiple complex needs
Isolated
Not linked in with any other services
CCP are an outreach service & therefore meet clients in their own environments:
i.e. parks, rooming houses, low cost private hotels, drop in centres & other places.
Support, information & assistance with referral to other appropriate services.
