Insecure Housing:
Short term support with housing, social & health issues. People 50+ within Port Phillip or Stonnington with complex care issues who live in Insecure housing eg;
 pension-level SRS
 rooming houses
 community housing
 private motels
 transitional housing
Community Connections Program (CCP):
Aged 50+ experiencing or at risk of homelessness or living in insecure or low-cost housing:
 multiple complex needs
 Isolated
 Not linked in with any other services
 CCP are an outreach service & therefore meet clients in their own environments:
i.e. parks, rooming houses, low cost private hotels, drop in centres & other places.
 Support, information & assistance with referral to other appropriate services.
Access and Support diversity team:
Case management for people 50+ with complex needs whose diversity characteristics include:
 Indigenous
 GLBTI
 dementia
 People residing in Insecure Housing or are homeless
