The Short-Term Restorative Care (STRC) Programme is delivered in the form of goal oriented, high intensity and multidisciplinary package of services, aiming to optimise the functioning and independence of older people and to minimise functional decline.

Able to achieve self- identified goals with the help of our multidisciplinary team (MDT)
Able to regain or maintain functional strength and balance, cognitive and psychosocial functioning in 8 week time
Able to manage activities of daily living more safely around the home
Able to self-manage interventions after education and strategies provided by our MDT
Multicultural Care
Wellness, reablement and restorative approaches are emerging as powerful ways to help older people improve their function, independence and quality of life. The Short-Term Restorative Care (STRC) Programme aims to reverse and/or slow ‘functional decline’ in older people and improve their wellbeing.
BlueCross Western
Short Term Restorative Care (STRC) provides early intervention care aimed at improving and optimising the independence of older people. BlueCross currently offers STRC for people in their own home, aged care residence or a combination of both, in the Western Metro region of Victoria.
Programmed Care
Programmed Care provide a wide range of nursing care, therapy services, Home Care Packages and rehabilitation programs including Short Term Restorative Care (STRC) program.
STRC is a short term, 8 week program to reduce the possibility of requiring hospital and assist you to regain functional capacity and will support you to reach your goals to improve/maintain your independence to remain at home.
Services can include but are not limited to Physiotherapy, Occupational Therapy, Nursing services and other Allied health services. The need for personal care assistance, shopping /cleaning assistance, Home modifications and equipment aids will be assessed and a care plan commenced as per your individual needs. STRC program can help you improve your balance, strength and endurance to improve mobility and maintain activities of daily living.
Programmed Care can assist you with planning long term community services.
The District Nurses
The District Nurses provide short-term restorative care (STRC) in the North and South of Tasmania.
The District Nurses STRC is a goal oriented, multidisciplinary package of care designed to meet the client’s needs to help them stay at home and help defer entry into higher levels of care.
STRC is delivered by a care team of at least three health professionals, one of which will be a doctor (usually the clients GP).
The District Nurses specialist RN’s will coordinate the clients STRC, and the package will not affect any services the client currently receives.
The District Nurses do not charge clients fees for STRC packages.
The types of services a client may receive include, but is not limited to:

• physiotherapy
• occupational therapy
• footcare
• clinical nursing
• personal care and assistance
• meal preparation, nutrition and diet advice
• personal medical alarm

The EACH Short-Term Restorative Care Program provides a flexible, customised service to improving wellbeing and regaining skills to live independently in the community. The program serves the Eastern suburbs and runs over an 8 week period. Your Case Manager will tailor and coordinate services with our experienced health team, and liaise with your GP. You can be confident that you will receive the best of care from our supportive, caring team.

In consultation with your GP, Suncare delivers STRC over an eight-week period and can be in the comfort of your own home.
The program is designed to get you back on your feet, to help get you moving and to feel strong, healthy and happy.
There are a wide range of services available designed to meet your individual needs, including:

• Psychologist or counsellor support
• GP or geriatrician advice and medical care
