Continence advisory service- Provides advice and education for people of all ages who experience bladder and / or bowel control issues. Cardiac rehabilitation - An eight-week program, recognised by the National Heart Foundation, provides an education & rehabilitation program for clients who have suffered a cardiac event and for their partners. The program includes a moderate individual exercise program and a related health-topic presentation. Falls Prevention Program – the program is designed to help people who feel they may be at risk of falling or have suffered falls already to feel more confident and remain independent and upright.

Dementia Australia ACT - Specialised Support Services
Dementia Australia will no longer be accepting referrals for this service through the My Aged Care portal. However, this program will continue under different funding arrangements, with changes to program eligibility, including:
• Accessibility across all of Australia
• No restrictions for people who have other funding packages in place
• Availability to a wider range of ages
• No client contributions

Dementia Australia’s individualised dementia support includes a mix of direct services, tailored support and expert advice. These services are for people living with dementia, mild cognitive impairment, or individuals seeking a diagnosis (and their carers).

Access and Support Hume City Council
Access and Support works with people who are eligible for the Commonwealth Home Support Programme to access aged care services to improve their wellbeing and ability to live in the community as independently as possible.
The Access and Support Officer provides short-term episodic support to people:
- From the Aboriginal and/or Torres Strait Islander community
- From culturally and linguistically diverse (CALD) backgrounds
- Experiencing financial or social disadvantage
- Veterans
- Experiencing homelessness, or at risk of homelessness
- From the lesbian, gay, bisexual, intersex or transgender communities
- Care leavers
- Parents who have been separated from their children by forced adoption or removal
