Dementia Australia will no longer be accepting referrals for this service through the My Aged Care portal. However, this program will continue under different funding arrangements, with changes to program eligibility, including:
• Accessibility across all of Australia
• No restrictions for people who have other funding packages in place
• Availability to a wider range of ages
• No client contributions

Vision Australia provides a full range of services and supports to people who are blind or have low vision. Our services support you with techniques for reading including use of computers; advice and training to remain independent in the home and with personal care, and training on strategies to help move about safely and accessing public transport and more; and services to help you connect with others
Central Bayside Community Health Services
Our Dementia Support Service is a specialist service which assists people with cognitive decline or dementia to remain living independently at home. The service focuses on wellness, reablement and restorative care approaches with clients and carers. The service works closely within a team of allied health professionals to provide coordinated care.
The service is available at our centres, via Telehealth (phone or video consultation) or at clients’ homes.
