• Home safety Ax (grab rails, bannister rails, steps, ramps)
• Equipment prescription – Low Vision products
• Falls prevention
• Daily skills programs (ie; cooking)
• Assisting in community engagement and meaningful activities
Counselling (individual)
Adaptive technology training (not cost of equipment)
Education to other services providers to support an individual
Volunteer Support Services (visiting, accompanied shopping and social support in the community)
Low vision specific group activities (short term – Peer support)

Dementia Australia will no longer be accepting referrals for this service through the My Aged Care portal. However, this program will continue under different funding arrangements, with changes to program eligibility, including:
• Accessibility across all of Australia
• No restrictions for people who have other funding packages in place
• Availability to a wider range of ages
• No client contributions
