Sydney Community Services - Continence Advisory
provides information and support for people aged 65 or older and Aboriginal & Torres Strait Islander people aged 50 or older who need aged care services to maintain their wellbeing and independence at home by:
Assisting people who are vulnerable or disadvantaged including those who don’t have computer literacy or family or friends who can help, to understand and navigate the My Aged Care system.
Assisting carers, family and representatives who want information about how the My Aged Care system works and how to proceed.
Providing ongoing support and monitoring, with consent, following an assessment for Commonwealth Home Support Services or a Home Care Package.
The service is free and can be provided in a number of ways - on the phone, at Sydney Community Service’s Lane Cove office (accessible parking underneath the building), via email and in the home.

Footprints Community
Footprints offers a free specialist dementia advisory service to provide support for people (and their carers) who wish to remain living independently in their home and communities. We promote awareness of dementia, provide information, education, and support. We can assist you to maintain your independence by supporting you to navigate the My Aged Care website to access support services for your daily living needs. As well, we can visit your home to identify and recommend aids to help in making it “dementia friendly”. We can provide further support by:
• Contacting My Aged Care and supporting you with the assessment process
• Helping you understand how CHSP services work
• Introducing services that will assist with enhancing your quality of life
• Accessing Nursing and Allied Health Options to address changing needs
• Connecting with our Social Support Groups
• Participating in community activities i.e. scenic drives, visit local cafes.

Wintringham Assistance with Care and Housing (Western - Lionsville)
Wintringham is a specialised welfare company that provides housing and care to elderly, frail men and women who are homeless or at risk of homelessness. In addition to the 650 Home Care Packages Wintringham provides around Victoria, we are able to assist with the following services: · Specialist team providing over the phone advice and information about housing and support options · Access and Support Services providing short term assistance by linking people to services and or healthcare providers · Outreach Teams providing linkages to services plus assistance to find affordable, long term housing · Recreation Programs and activities aimed at enhancing community inclusion and participation · Support to maximise health, wellbeing and to maintain tenancy · Assistance with activities of daily living and healthcare within a full care accommodation setting.
