The WA Country Health Service (WACHS) is a State Government organisation providing comprehensive health services to country West Australians.
WACHS spans 2.55 million square kilometres and is the largest country health system in Australia covering the whole of the State outside the metropolitan area.
Despite vast distances, WACHS provides health services to residents and visitors that include emergency and hospital services, population, public and primary health care, mental health, drug and alcohol services, Aboriginal health, child and community health and residential and community aged care services.
WACHS provides aged and community care services for older people across our communities, including aged care assessments, community care, residential aged care, subacute and acute care, and transitioning older people from hospital back into the community.
WACHS is a provider of CHSP services such as domestic assistance, personal care, meals and food services, and transport.

EACH Social Activity Group - Hobart
EACH Social Activity Groups offer the opportunity for older people to continue to be active within the community. The groups provide social and recreational activities in a supported and inclusive environment. Staff will plan daily activities with the participants based on people's interests as well as their needs. These may include celebrating community events, gentle physical exercises, reminiscing activities, music, guest speakers and creative art.

The PAAC’s encourage residents to connect with people in their local community
while enjoying good company, social activities, healthy and active ageing programs, social outings and

Monash Seniors on the Move is a door-to-door social outings program that takes you to various local and
regional destinations. Meet new people and make new friends while getting out and about.
The Halcyon Social Support Centre is a specialist day program for people living with Alzheimer’s
disease, dementia/memory loss and mobility restrictions.
EPIS Tom Price
EPIS provides group excursions and activities both in and away from the centres. Clients can enjoy trips out of town to waterholes, rivers and larger towns to allow them to engage in activities such as fishing, camp fires and bush medicine. Inside activities are organised when the weather is conducive. These might include painting, arts and crafts, gentle exercise, music, bingo, board games and yarning about old photos.

Jubilee Community Care is a not for profit aged care service provider, established in 1989.
We are a local service provider to your area with our offices based in Indooroopilly.
We provide care and support services enabling people to enrich their life by providing personalised services.
We will organise your care and support services on the day(s) and time(s) of your choosing and with regular Support Workers.
Our Support Workers are employed on permanent contracts of employment, reference checked, have a Police Clearance Certificate, attend annual and on-going training including First Aid and CPR, and are supervised by experienced Client Support Coordinators.
Client of Jubilee Community Care can access hydrotherapy and exercise classes, falls prevention programs and weekly social activities and events connecting you with your community.
