The WA Country Health Service (WACHS) is a State Government organisation providing comprehensive health services to country West Australians.
WACHS spans 2.55 million square kilometres and is the largest country health system in Australia covering the whole of the State outside the metropolitan area.
Despite vast distances, WACHS provides health services to residents and visitors that include emergency and hospital services, population, public and primary health care, mental health, drug and alcohol services, Aboriginal health, child and community health and residential and community aged care services.
WACHS provides aged and community care services for older people across our communities, including aged care assessments, community care, residential aged care, subacute and acute care, and transitioning older people from hospital back into the community.
WACHS is a provider of CHSP services such as domestic assistance, personal care, meals and food services, and transport.

Resthaven supports those who are at risk of homelessness or unable to receive the aged care supports they need as a result of hoarding behaviours or circumstances, or difficulty keeping their living environment safe and clean. Our team is sensitive to the person’s needs and take the time needed to build rapport and trust.
Hoarding can be associated with health risks and can impact on a person’s friends and family. It can also place the person’s housing at risk if they are living in an environment that is unclean. Assistance with Care and Housing - Hoarding and Squalor services may include:
• developing a plan with the person
• one-off clean-ups
• reviewing care plans
• linking clients to specialist support services.

Resthaven Community Services - Paradise and Eastern
Assistance with Care and Housing: provides assistance to older people who have tenuous housing arrangements (homeless or at risk of becoming homeless) to find secure and affordable housing and link with support services. The client’s specific needs and circumstances are discussed and an appropriate plan of assistance is designed with the client. Trained staff are sensitive to the individual circumstances of each person. In addition to supporting the client to find housing, services include assistance through advocacy, advice/information, and linking with services such as in-home care, community groups, and health services.
