IRT Assistance with Care and Housing
Through the Program, the Coordinator helps seniors link to suitable community services, working with them to find sustainable and affordable housing. This support includes: • locating suitable and affordable long-term housing • providing advice and assistance on housing applications • helping people to access accommodation related legal and financial services • assistance with packing, moving and relocating seniors into their new home • accessing relevant local community support to help improve quality of life • where suitable, advocating and negotiating with services • linking and referring seniors, where appropriate, to aged care, community care, healthcare and welfare services, disability services. Those who can access the IRT ACHA Program include seniors who are: • living on low incomes • living in unsuitable accommodation (for example, expensive housing or accommodation which restricts their access to support services) • homeless, or at risk of becoming homeless

Hutt St Centre - ACHA
Hutt Street Centre provides an Assistance with Care and Housing program to assist people who are homeless and at risk of homelessness due to inadequate and/or insecure housing and/or socially and financially disadvantage. The program aims to reduce the impact of homelessness through assessment, referral, linking in and advocacy support so that the person is able to access and sustain accommodation and optimise their independence at home and in the community.
Hutt St Centre - Homelessness Services
Hutt Street Centre provides an Assistance with Care and Housing program to assist people who are homeless and at risk of homelessness due to inadequate and/or insecure housing and/or socially and financially disadvantage. The program aims to reduce the impact of homelessness through assessment, referral, linking in and advocacy support so that the person is able to access and sustain accommodation and optimise their independence at home and in the community
Footprints Community
Footprints Assistance with Care and Housing (ACH) program is designed to support people who are homeless or at risk of homelessness, to access appropriate and sustainable housing as well as community care and other support services, specifically targeted at avoiding homelessness or reducing the impact of homelessness. ACH also provides support to people experiencing symptoms of hoarding disorder or hoarding related behaviours.

The service has a housing support worker available in Warrnambool on a fortnightly basis on Mondays 12pm-5pm and Tuesday 9am-1pm or by appointment. the worker is located at the Archie Graham Community Centre, 118 Timor Street, Warrnambool.
