This service is for Continence Nursing only.

Service delivery once every two months.
Life Care North/East
Nursing care (monitoring and treatment of medically diagnosed clinical conditions) is provided by a registered or enrolled nurse. Tasks such as recording client clinical observations may be conducted by trained and skilled staff, overseen and supervised by a Registered Nurse or Enrolled Nurse.
Nursing care can include: continence assessment, education and advice; regular monitoring of BSL, blood pressure etc; education in self-management of chronic conditions; short term wound care; and support with medication management.
Northern Territory PHN - Byronlink - Continence Nurse
Outreach service delivery - provides continence nursing services to remote Central Australian communities. Frequency - bimonthly. Is a DEP prescriber.

Uniting AgeWell Strathdon Home Care Packages
At Uniting AgeWell (UA) our focus is on supporting older people to experience a sense of wellbeing, choice and independence at every age and stage of their lives; to feel valued members of their community; and to have easy access to the services they need to live the life they want. It’s what we call ageing well. UA Home Care Packages provide the support and services you need to remain living at home, safely and independently for as long as possible. Funded by the Australian Government, these packages can help you keep doing the things that are important to you, that bring you pleasure and help you stay connected to your community. Several levels of support are available and can assist people with either low level or high level care needs, including dementia. Level 2 packages are for people with low level care and support needs.
Helping Hand Metro Community
Our Community Nurses can assist you with, but not limited to:
• Individual nursing assessments and care plans
• Skin integrity and wound-care management
* Blood pressure monitoring and BGL monitoring in conjunction with clients GP
• Continence management, female catheter care
• Medication supervision/prompting and management, including subcut and Intramuscular injections
• Assistance to manage PEG and stoma’s
• CAPS applications, dementia supplement applications and oxygen supplement applications post appropriate nursing assessments
• Referrals to allied health as required
