The WA Country Health Service (WACHS) is a State Government organisation providing comprehensive health services to country West Australians.
WACHS spans 2.55 million square kilometres and is the largest country health system in Australia covering the whole of the State outside the metropolitan area.
Despite vast distances, WACHS provides health services to residents and visitors that include emergency and hospital services, population, public and primary health care, mental health, drug and alcohol services, Aboriginal health, child and community health and residential and community aged care services.
WACHS provides aged and community care services for older people across our communities, including aged care assessments, community care, residential aged care, subacute and acute care, and transitioning older people from hospital back into the community.
WACHS is a provider of CHSP services such as domestic assistance, personal care, meals and food services, and transport.
St Vincents At Home Nursing
St. Vincent’s At Home (SVAH) provides community nursing to patients within several Local Government Areas (LGA) and has a unique service model with funding from various sources including: CHSP, Home and Community Care Program for Younger People (HACC-PYP), Hospital in the Home (HITH), Post-Acute Care (PAC), Fee for Service (including TAC and Workcover), and is a Community Nursing Provider for Department of Veteran Affairs (DVA). In addition, our Access and Support (A&S) Service is predominantly a direct support role, to assist people who are eligible for HACC/CHSP services to access these, and other community services.

Eastern Shore Community Nursing
operates within a primary health care framework to deliver nursing services that support clients to remain independent and develop a self-management capacity to maintain their health and wellbeing within the home environment.
Community nursing services are available in clinic settings at Clarence Integrated Care Centre or home visits for clients unable to attend a clinic.
Community Nursing services include but are not limited to -
assessments and development of agreed plans of care,
Catheter care and continence,
Chronic condition/self-management programs and support,
DVA Community Nursing Program,
Health education and promotion,
Hospital Avoidance Program,
Complex Medication Management,

Community Health Nursing Service provides nursing care at a Community Health Centre clinic. Clinical nursing interventions are attended to based on health care needs.
