Nutrition & Dietetics - Hornsby
Community dietitians provide a range of nutrition services including dietary assessment and specialist dietary advice aimed towards the prevention of malnutrition, improving nutritional status and therapeutic management of many chronic diseases and conditions.
Specialist nutrition advice will be provided to clients and/or their carers to address issues such as inadequate oral intake, unintentional weight loss or weight gain and provide special diet plans to manage their condition/symptoms. For example diabetes, cardiovascular conditions, kidney disease and dialysis, food and nutrition advice for dementia and chronic diseases.
Recommendations may include diet modification, nutrition plans, including oral nutrition supplements and enteral feeds to the client and their carers. Interpreters are available if required.

Service targets consumers who require short-term Occupational Therapy assessment and/or intervention for the safe provision of the Whitehorse City Council Commonwealth Home Support Programme (CHSP) services and maximising consumer independence. Expected wait list time is 5 months.

Dhelkaya Health - Allied Health
CHSP funded allied health services include Podiatry and Occupational Therapy. Podiatry offers clinic based services and home visits when a person can't leave their home. Podiatry works with Mount Alexander Shire to provide the In Home Foot Care Program. Which provides foot care for people with low risk feet. In late Autumn and Winter Wax Bath groups are held for people at risk of chill blains. Physiotherapy can provide assessments to support people attend activities. A Falls prevention maintenance Group is run on Fridays called Steady Feet this allows people who have been through a falls program to be involved in falls specific exercise program following their falls program.
Occupational Therapy works to support people to stay in their homes and maximize independence using a strength based approach. They are able to partner with Mount Alexander Shire to plan for property maintenance.
