Forest Hill AgeWell Centre Allied Health program offers the following disciplines. Programs are offered at 9 Jolimont road, Forest Hill and some select Physiotherapy programs are also offered at 79 Thames street in Box Hill. Occupational Therapy is offered in home.
-Physiotherapy – for recent injuries, illnesses, new diagnosis and hands on treatment
-Exercise Physiology – for strengthening, balance, conditioning, falls prevention and all exercise programs
-Occupational Therapy – for in home safety assessments, ADLs assessment and equipment prescription
-Speech Pathology – for speech and swallowing disorders
-Podiatry – for footcare and footwear assessment and intervention
-Hydrotherapy – located at Box Hill Aqualink pool. Assessment required by Exercise Physiology before attending

As a result we are now offering 1:1 allied health support via individual consultations in your home for occupational therapy, physiotherapy and exercise physiology.

Home Nursing Group provides community aged care services to support you in the home you love, surrounded by family and friends and connected with your community. Home Nursing Group can provide access to Allied Health and Therapy Service delivered by qualified professionals at the their consulting rooms.

Home Nursing Group provides community aged care services to support you in the home you love, surrounded by family and friends and connected with your community.
Home Nursing Group can provide Social Support to assist you to participate in community life, with activities such as:
• Visiting services
• Telephone based monitoring services
• Assisting with shopping and other related activities
• Social outings, which may be provided to more than one person
• Accompanied activities
The service you receive is based entirely on your needs and is flexible enough to change over time as your requirements change.

Based on an individual assessment, qualified, registered allied health professionals work with the client to establish a tailored plan of treatment that may include individual or group therapy, exercises, self-management of chronic conditions. Services are provided by or under supervision of allied health professionals, with a preventative, maintenance, or restorative focus, to support the client to be independent. Services include:• Podiatry • Occupational therapy • Physiotherapy • Formal counselling • Education in use of equipment • Exercise/gym classes/falls prevention • Tai chi, Chi Ball • Aquacise. • Condition-specific (Parkinson’s, stroke/pulmonary conditions, arthritis, diabetes) self-management. Sessions are conducted at Elizabeth with some services (e.g. Occupational Therapy) provided in the person’s home. Transport to and from venue and personal assistance may be provided, depending on the person's circumstances.
