Access HC and Inspiro have merged. Access HC Belgrave offers Dietetics, Diabetes Nursing, Physiotherapy, Exercise Physiotherapy, Podiatry and Occupational Therapy (home based). Due to staffing, OT, Podiatry and Physio are experiencing high demand. These services are restricted to clients meeting clinical urgency criteria (or semi urgent criteria for OT). For podiatry, clients require a history of foot wounds, or current bone fracture. For Physio/OT, please ensure referrals clearly outline:
• Diagnosis, information regarding any terminal/ palliative or progressive conditions
• Risks associated with pressure care
• Falls – where they occurred, the cause, injuries sustained, assistance and medical input required
• Safety and independence with transfers and mobility
• Cognition
• Existing supports – who they live with, carer information, the sustainability of these arrangements, risks to carers
• Any risks associated with capacity to remain at home

Hobart AgeWell Centre Allied Health program offers the following disciplines. Programs are offered at 92-94 Main Road, Moonah. Occupational Therapy is offered in home.
-Physiotherapy – for recent injuries, illnesses, new diagnosis and hands on treatment
-Exercise Physiology – for strengthening, balance, conditioning, falls prevention and all exercise programs
-Occupational Therapy – for in home safety assessments, ADLs assessment and equipment prescription
--Podiatry – for footcare and footwear assessment and intervention
-Remedial massage – for pain management
