Referrals must meet the following criteria to be accepted. If the following criteria is not met, referrals will be rejected.
1. The referral must specify “Occupational Therapy” as the service sub-category.
2. Referrer must only complete one referral, completing multiple referrals to occupational therapy does not increase the speed at which a client is processed.
3. Reason for referral must be included in “referral comments”. Link Health & Community service coordination team will not be viewing support plans/comprehensive assessments to gather more information about a referral. If the criteria for referral is not met in “referral comments” the referral will be rejected requesting “further information to be added”.
