Referrals received through My Aged Care must meet the following criteria to be accepted. If the following criteria is not met, referrals will be rejected.
1. The referral must specify “Physiotherapy” as the service sub-category.
2. Reason for referral must be included in “referral comments”. Link Health coordination team will not be viewing support plans/comprehensive assessments to gather more information about a referral. If the criteria for referral is not met in “referral comments” the referral will be rejected stating "further information to be provided"
