Short-term Restorative Care (STRC) is an eight-week program designed to help clients recover from injury or illness, improve their wellbeing, and help them stay independent. STRC achieves this through the delivery of a time-limited, goal-oriented, multi-disciplinary and coordinated range of services designed for, and approved by, the client. As a client, you will work with a team of health professionals to work out what you want to achieve and the best ways to get there.
There are a wide range of services and activities available as part of STRC. These include one-on-one allied health services, weight management, pain management, group exercise classes, yoga, hydrotherapy classes, and much more..
To be eligible, you must be slowing down mentally and/or physically, need help with everyday tasks, and want to stay independent. This program is ideal for you if you would like to improve your health and wellbeing, or would like assistance to recover from an injury or illness.

Wellness, reablement and restorative approaches are emerging as powerful ways to help older people improve their function, independence and quality of life. The Short-Term Restorative Care (STRC) Programme aims to reverse and/or slow ‘functional decline’ in older people and improve their wellbeing. A new form of flexible care, the STRC Programme, was established to increase the care options available to older people, through a time-limited, goal-oriented, multi-disciplinary and coordinated package of services. Each care package is designed with and agreed to by the client, and may be delivered in a home setting, a residential setting, or a combination of both. Any individual wishing to receive STRC will need to be referred for an Aged Care Assessment Team (ACAT) assessment through My Aged Care. STRC services are:
HammondCare STRC - South West Sydney
HammondCare STRC Programmes are designed to support people who are 65 years or older and are living at home in the SWS Area. The STRC Programme is an early intervention programme that offers both preventative and targeted care, aimed at delaying functional decline or targeted at specific functional goals such as safe mobility and falls prevention.
Programmes are delivered by a multi-disciplinary team (Allied Health, Medical and Care staff) with experience in designing and delivering water and land based restorative care programs for older adults. Programmes are delivered over a period of (up to) 8 weeks with the focus of each program varying depending on the individual clients’ goals and care needs. Services may be delivered in either, or both, a home care or a residential setting.
