HammondCare STRC - South East Sydney
HammondCare STRC Programmes are designed to support people who are 65 years or older and are living at home in the SES Area. The STRC Programme is an early intervention programme that offers both preventative and targeted care, aimed at delaying functional decline or targeted at specific functional goals such as safe mobility and falls prevention.
Programmes are delivered by a multi-disciplinary team (Allied Health, Medical and Care staff) with experience in designing and delivering water and land based restorative care programs for older adults. Programmes are delivered over a period of (up to) 8 weeks with the focus of each program varying depending on the individual clients’ goals and care needs. Services may be delivered in either, or both, a home care or a residential setting.
HammondCare’s Centre for Positive Ageing actively promotes restorative care programs for older people in the local community and to service providers.
Enrich Living Services
Enrich Living is an approved Short-Term Restorative Care provider who will work with you to deliver a tailored package of care and services to achieve your set goals. STRC is delivered by a Multidisciplinary Care Team of three or more specialist care providers from different disciplines who will collaborate to provide you with comprehensive, outcome-focused treatment.

EACH Short Term Restorative Care - South West Sydney
The EACH Short-Term Restorative Care Program provides flexible and customised services to improve people's wellbeing and regain skills to live in the community as independently as possible. The program runs over an 8 week period with a Case Manager who tailors and coordinates services with our experienced health team, and liaises with your GP. You can be confident that you will receive the best of care from our supportive and caring team.Examples of supports that can be provided to achieve improved independence include: physiotherapy, occupational therapy, nutrition, nursing, personal care, home modifications, and if helpful things such as assistive technologies. We work with you to create the best plan to enable you to continue living safely and independently. The District Nurses
The District Nurses provide short-term restorative care (STRC) in the North and South of Tasmania.
The District Nurses STRC is a goal oriented, multidisciplinary package of care designed to meet the client’s needs to help them stay at home and help defer entry into higher levels of care.
STRC is delivered by a care team of at least three health professionals, one of which will be a doctor (usually the clients GP).
The District Nurses specialist RN’s will coordinate the clients STRC, and the package will not affect any services the client currently receives.
The District Nurses do not charge clients fees for STRC packages.
The types of services a client may receive include, but is not limited to:
• aids and equipment
• physiotherapy
• occupational therapy
• footcare
• clinical nursing
• personal care and assistance
• meal preparation, nutrition and diet advice
• personal medical alarm

The program is designed to get you back on your feet, to help get you moving and to feel strong, healthy and happy.

Feros Care (STRC) Darling Downs
Short-term Restorative Care (STRC) is an eight-week program designed to help clients recover from injury or illness, improve their wellbeing, and help them stay independent. STRC achieves this through the delivery of a time-limited, goal-oriented, multi-disciplinary and coordinated range of services designed for, and approved by, the client. As a client, you will work with a team of health professionals to work out what you want to achieve and the best ways to get there.
There are a wide range of services and activities available as part of STRC. These include one-on-one allied health services, weight management, pain management, group exercise classes, yoga, hydrotherapy classes, and much more..
To be eligible, you must be slowing down mentally and/or physically, need help with everyday tasks, and want to stay independent. This program is ideal for you if you would like to improve your health and wellbeing, or would like assistance to recover from an injury or illness.
