Wellness, reablement and restorative approaches are powerful ways to help older people improve their function, independence and quality of life.
STRC is an early intervention program that aims to reverse and/or slow “functional decline” in older people and improve wellbeing, through the delivery of a time-limited, goal-orientated, multi-disciplinary and coordinated range of services designed for and approved by you. It is ideal for people who have the desire to return to earlier or improved level of independence and who are not currently receiving any Commonwealth Aged Care Services such as a Home Care Package. Access to STRC is due to a functional decline resulting in you needing assistance for a short period of time (maximum 8 weeks),
