We provide access to a range of residential and community aged care services and dementia care in various country locations, either wholly or in partnership with the Australian Government. These services are largely delivered through our hospitals (as part of our Multi-Purpose Services).

Barwidgee Lodge is a 30 Bed residential aged care facility in Myrtleford, North East Victoria and forms part of the overall Alpine Health Multi Purpose Service. Barwidgee Lodge provides permanent and respite care in single room accommodation.

This program provides short-term care and services for older people who have been dischargedfrom hospital but require extra support. Transition Care Program allows you to continue yourrecovery out of hospital while long-term care is arranged.To receive Transition Care Program services you are assessed and approved for care by theAged Care Assessment Service while you are in hospital.Transition Care Program can be provided in an aged care setting, or in your own home. Thisdepends on the type of care you need. You may move from one setting to another.Transition Care Program starts as soon as you leave hospital. Most people stay on the programfor six to eight weeks. This depends on your care needs and goals. The maximum is 12 weeks.In this time you and your Care Co-ordinator will arrange suitable long-term support.
