St Vincents Specialised Support Service
The Access and Support Service works directly with people from diverse backgrounds and their carers, to access a wide range of community based services. We can talk with you about your needs and goals, make referrals, facilitate registration and assessments as required. Types of community services we can assist you access include: My Aged Care, Commonwealth Home Support Program (CHSP), National Disability Insurance Scheme (NDIS), housing services, social support groups, home-based or residential respite, Home Care packages, Nursing, and Allied Health services. Access and Support also takes into account other diversity considerations such as people living with dementia, age, gender, faith and spirituality.
CatholicCare Sydney
Short term support available to people with vision, hearing or cognitive impairment to regain or increase independence. Clients can also join Wellness group for weekly exercise physiology or fall prevention program.
Volunteering Western Victoria - Nhill
Volunteering Western Victoria is able to assist with a variety of specialised services such as Dementia advisory assistance through our Memory Lane Café’s and Memory Strategy Groups. These groups assist you with navigating your way through the confusing choices you face with your medical or care needs. They also provide the opportunity for you to socialise and seek peer support from those that understand. Guest speakers provide the opportunity for you to gain memory and support strategies.
Casey City Council
Specialised advisory service for people:
- aged over 65 living at home with a clinical condition and/or specialised need
- aged over 50 for Aboriginal and Torres Strait Islander people
- people who are experiencing homelessness
The aim of the program is to assist people to build knowledge of services and supports to improve their health and wellbeing.
The service is entry-level and intended to support individuals, their families and carers. It provides a holistic approach to assist people to access advocacy support and receive information to build capacity and to remain living in their own homes.
The team will assist to:
- develop client-centred goal plans
- provide advocacy to improve access to services and build connections
- support care coordination sessions to assist wellbeing
- conduct client reviews and monitor progress
Wyndarra Centre Inc.
SMRC Specialized Support Services
Formerly known as Access and Support, this program provides individual support for CALD elderly to access services. It includes initial needs identification, referral, follow up and advocacy for CALD elderly leading to improved opportunities for service access. We are not a continence or dementia advisory service, or a vision or hearing service.
City of Whittlesea - Access and Support
The Access and Support role assists people with complex needs due to their diversity to access services that will improve their capacity to live in the community as independently as possible. The role provides short-term episodic support.
It is not a case management role.
