Chorus Perth South West Metro
At Chorus we want to know what matters to you and how we can help. We believe that sometimes it’s the little things that make a big difference. We offer more than just a service – we walk by your side to help you with the things that are important to you.
Chorus listens. If you need help in your home or garden, getting out and about, or would like to take part in social and wellness activities, our team would love to have a chat about how we can partner with you.
If you require transport, we’ll get you safely and efficiently from A to B. If looking your best matters to you, we’ll help with that. If preparing a shopping list is important, Chorus is here to help. If there are activities you’ve always enjoyed (or would like to try) we’ll do our best to make them happen.
Phone Chorus on 1800 264 268, email hello@chorus.org.au, or visit chorus.org.au to find out how we can help you live the life you choose. Chorus is a merger of Volunteer Task Force, Care Options and Community First.

Parkinson's SA
Parkinson's SA provides specialised support, information and education to people with Parkinson's disease and other movement disorders and their family carers. Social Workers provide one on one counselling and support by phone or face to face and can inform people of and refer people to other community programs. These include Parkinson's SA seminars and specialised educational sessions which aim to encourage self management and independence. Support Groups in metropolitan and country areas provide a mutual aid model of care that gives people the chance to come together with others in the same situation, usually once a month. The organisation will advocate for people on an individual and systemic level.

Lifebridge Australia Ltd - CHSP
Offering assistance to carer's and their families to manage and maximise independence and enablement to continue living in their own homes. This may consist of tailored support and advice including dementia support and also direct service delivery. Carer Support Meetings are a part of this service along with our Coffee and Chat held monthly on the third Thursday of the month and first Thursday of the month. Carer Support Groups provide the opportunity to develop peer support relationships and take some time out over lunch. Participation in a carer support group can help sustain ongoing carer relationships through information sharing. Everything discussed is strictly confidential. These services can assist carers and families to manage conditions and challenges associated with the caring role. These services are provided on a transitional basis and also include customer advocacy.
