Connections Program
The Connections program provides outreach to people who are homeless or at risk of homelessness and who have complex unmet health needs. Work typically occurs on public housing estates, rooming houses and other low cost accommodation settings.
Criteria is singles and couples on low incomes who are experiencing housing issues and have complex health and or social needs.
Volunteering Western Victoria - Nhill
Volunteering Western Victoria is able to assist with a variety of specialised services such as Dementia advisory assistance through our Memory Lane Café’s and Memory Strategy Groups. These groups assist you with navigating your way through the confusing choices you face with your medical or care needs. They also provide the opportunity for you to socialise and seek peer support from those that understand. Guest speakers provide the opportunity for you to gain memory and support strategies.
Casey City Council
Specialised advisory service for people:
- aged over 65 living at home with a clinical condition and/or specialised need
- aged over 50 for Aboriginal and Torres Strait Islander people
- people who are experiencing homelessness
The aim of the program is to assist people to build knowledge of services and supports to improve their health and wellbeing.
The service is entry-level and intended to support individuals, their families and carers. It provides a holistic approach to assist people to access advocacy support and receive information to build capacity and to remain living in their own homes.
The team will assist to:
- develop client-centred goal plans
- provide advocacy to improve access to services and build connections
- support care coordination sessions to assist wellbeing
- conduct client reviews and monitor progress

Care at Home provides Diabetes Education and a Continence Service for people living in the Hepburn Shire and Macedon Ranges Shire. Our Diabetes Nurse Educators and Continence Nurse Consultants, all offer specialised comprehensive assessments, management and treatment to meet the client's goals of care. We offer home visits or clients can attend an appointment at one of our five CHRH campuses (Kyneton, Daylesford, Clunes, Creswick, Trentham)
