BALLARAT Memory Nurse - Grampians Health
The Memory Support Nurse will provide practical and emotional support to anyone living with Dementia or Memory Issues. The service is aimed at:
- Maximising your independence to stay living in your home for as long as you can and wish to do so
- Completing Clinical assessments to attend Grampians Health Eyres House and Midlands PAGs/Social Support Groups
- Delivering nursing component of COPE - Care of People with Dementia in their own Environment (also requires Occupational Therapy referral)
