These services help clients, and their carers and families, to manage these conditions and maximise client independence to enable them to remain living in their own homes.

provides information and support for people aged 65 or older and Aboriginal & Torres Strait Islander people aged 50 or older who need aged care services to maintain their wellbeing and independence at home by:
Assisting people who are vulnerable or disadvantaged including those who don’t have computer literacy or family or friends who can help, to understand and navigate the My Aged Care system.
Assisting carers, family and representatives who want information about how the My Aged Care system works and how to proceed.
Providing ongoing support and monitoring, with consent, following an assessment for Commonwealth Home Support Services or a Home Care Package.
The service is free and can be provided in a number of ways - on the phone, at Sydney Community Service’s Lane Cove office (accessible parking underneath the building), via email and in the home.
