Guide Dogs SA/NT
Guide Dogs SA/NT provides a specialist vision service for children and adults with vision loss or blindness, enabling independence, participation and inclusion.
Our experienced specialist team of Occupational Therapists and Orientation and Mobility experts conduct clinical functional assessments and develop individual programs led by Client lifestyle and goals. Subsequent agreed vision related Occupational Therapy or Orientation and Mobility training can occur at home or in the community.

Orientation & Mobility - training programs to support safe, dignified, independent travel for people with vision loss of blindness

CCNB - Dementia Advisory Service
ccnb. has a Dementia Advisor who provides individual support to people with dementia and their families and carers. This includes education about dementia services available and general support and advice to deal with some of the challenges a diagnosis of dementia can present. Our Dementia Advisor works with service providers within the community to support the effective care of the person with dementia, as well as identifying and responding to the specific needs of carers and families. Our Dementia Advisor offers community awareness regarding risk reduction and dementia awareness.
Merri Health - Specialised Support (Dementia Advisory)
The Specialist Memory and Dementia Service, Merri Health provides assessment and intervention to clients with memory loss and dementia across all stages of dementia. Interventions are offered at the client’s home (for clients in the North & West catchment areas of Melbourne) or at the clinic at Glenroy. Services offered include: Occupational therapy (assessment & interventions for changed behaviours, home based environmental enhancement & design, memory strategies, activity planning, sensory modulation intervention, carer education regarding communication, home modifications & equipment prescription), Physiotherapy (gait aid prescription, mobility/balance assessments) exercise physiology (individualised exercise programs), speech pathology (communication assessment & intervention) and dietetics. Geriatrician and carer support/service coordination services are also available. Services are not time limited and clients can be re-referred to the service as required.
