Better Health Network - Parkdale
Our Dementia Support Service assists individuals with dementia and their carers in maintaining independence at home. We offer comprehensive assessments, personalised care plans, and coordinated care with other medical and allied health professionals. Additionally, we provide guidance on navigating My Aged Care, and help accessing community support services. Carers also receive education on managing dementia symptoms and gain access to a support groups.

Community Connections Program (CCP):
Aged 50+ experiencing or at risk of homelessness or living in insecure or low-cost housing:
multiple complex needs
Isolated
Not linked in with any other services
CCP are an outreach service & therefore meet clients in their own environments:
i.e. parks, rooming houses, low cost private hotels, drop in centres & other places.
Support, information & assistance with referral to other appropriate services.
Access and Support diversity team:
Case management for people 50+ with complex needs whose diversity characteristics include:

People residing in Insecure Housing or are homeless

Financially disadvantaged
