This program for people with dementia and/or memory loss and carers, can help you stay independent and remain in your own home for longer. We can do this by referring you to a wider range of services, organisations and essential supports, which may provide services such as home care, property maintenance and respite. We will work with you and your carers to create a care plan and achieve short-term goals over 12 weeks.

Community Connections Program (CCP):
Aged 50+ experiencing or at risk of homelessness or living in insecure or low-cost housing:
multiple complex needs
Isolated
Not linked in with any other services
CCP are an outreach service & therefore meet clients in their own environments:
i.e. parks, rooming houses, low cost private hotels, drop in centres & other places.
Support, information & assistance with referral to other appropriate services.
Access and Support diversity team:
Case management for people 50+ with complex needs whose diversity characteristics include:
