Social Outings
“Out & About” is a fortnightly social activity group for people who are over 55, high functioning, low physical support needs and independent in regards to personal care requirements. O & A focuses on ‘Fun, Food and Friendship’ and services residence living within the Brimbank Municipality. “O & A” is a great way to increase social activity whilst taking in the sights of Melbourne. The program is supported by Gateway’s door to door Community transport and members are collected from their own home, and returned directly home after the program. The group will get together between the hours of 9.30am and 3.30 pm including transport fortnightly on a Friday. Activities may include picnics or walks through parks and gardens, taking in a movie, visits to museums, exploring Melbourne’s wonderful food venues whilst enjoying lunch and good conversation with other members of the group. The choices are endless! Members of this group are encouraged to have an active say selecting the activities
NCN Health Numurkah Primary Health Services
Numurkah Network is a program that offers regular events that provide a friendly place to share a meal, catch up with friends or make new ones, and enjoy a range of activities that help to improve health and wellbeing. The program has a calendar of events that run every week on Wednesdays, Thursdays and Fridays. Most of our events cost $10.50 for Pension and Health Care Card Holders which includes morning tea and lunch. Some activities cost a little bit more. All prices are displayed on our event calendar. Other payment options are available. Referrals should be made via My Aged Care for clients 65 years or older (or 50 years and older for Aboriginal and Torres Strait Islander persons).
All other referrals should be made via our Intake Coordinator, via Connecting Care.
Planned Activity Groups Merri Outreach Support Service
Planned Activity Groups provide the opportunity for people who are isolated due to disability or age related issues to come together in a social environment and participate in activities supporting their ability to remain living at home in their communities. A nutritious meal is provided as part of each session.
At MOSS all our Planned Activity Groups are located on Department Older Persons estates in spaces shared by residents, who also are welcome for a drop in service.
Our aim is to offer physical activities that contribute to the health and well-being of clients, art & craft activities which encourage creativity as well as thinking activities that challenge clients cognitively in an environment in which participation is encouraged. We offer outings and engagement in community celebrations, keeping people connected to their local communities. Our spaces are welcoming and accepting of all individuals.

The WA Country Health Service (WACHS) is a State Government organisation providing comprehensive health services to country West Australians.
WACHS spans 2.55 million square kilometres and is the largest country health system in Australia covering the whole of the State outside the metropolitan area.
Despite vast distances, WACHS provides health services to residents and visitors that include emergency and hospital services, population, public and primary health care, mental health, drug and alcohol services, Aboriginal health, child and community health and residential and community aged care services.
WACHS provides aged and community care services for older people across our communities, including aged care assessments, community care, residential aged care, subacute and acute care, and transitioning older people from hospital back into the community.
WACHS is a provider of CHSP services such as domestic assistance, personal care, meals and food services, and transport.

Sydney Community Services - Social Support Group - Seniors Hub
This service provides social inclusion, enablement and community connection based supports for the elderly. SCS runs general senior’s groups, dementia support groups, ladies and men’s groups and day trip outing groups. This is a whole day service. Our staff are available to pick-up and drop-off clients at their own homes. Activities include:
• Music, craft, quizzes and games
• Picnics and BBQs
• Special events
• Day trips
• Gentle exercise classes.
MCCI Multicultural Friendship Groups - ACT and Queanbeyan
Multicultural Social Support Groups
Provided in a group-based environment and designed to develop, maintain and support social interaction and independent living.
Activities may include:
• Culturally specific group-based activities ( games, crafts and other interactive activities)
• Group excursions conducted by centre staff but held away from the centre
• Cultural meals or light refreshments
• Online group activities facilitated by the Bilingual Social Support Group Facilitators
We also provide other services including: Flexible respite, Social Support Individual, Domestic Assistance, Personal Care. Meals and Transport.
I addition we are an approved provider of aged care to deliver Home Care Package services.
MiCare Multicultural & Chinese Social Support Group Bentleigh
At our Multicultural and Chinese Social Support Groups, we look at the strength of the participant and empower them to participate in activity they choose. Our warm and welcoming social group will make people feel empowered and connected to the community. This will enhance their quality of life, maintain and improve their independence and to continue to live in the community with pride. All our staff are qualified and adopt person centre approach to support participants in the group. Activities include board games, physical games, Quizes, exercise, craft, themed days and outings. Baptcare Westhaven Day Centre

Our friendly staff will work with you to decide which options will best support you in your caring role. We offer a range of programs in our Social Support Groups that will help your loved one keep active, explore their interests and make and maintain friendships. Westhaven operates three times a week, offering regular bus excursions.
Our variety of programs include activities that celebrate cultural differences and meet the needs of people with Dementia by using Montessori techniques in a secure environment. We can also assist with medication administration and personal care requirements, as well as organise support groups and respite opportunities for carers.
