The ACHA program at cohealth provides short term support to people who are eligible for the ACHA program. The support provided may include:
• Advocacy in relation to issues with private and public tenancy,
• Support to those who are eligible to complete community and public housing applications
• Internal and external referrals to Allied Health Services.
• Support to access financial and legal services including elder abuse issues

Footprints Community
At Footprints our aim is to support people, including those with complex needs and challenging behaviours to remain independent and living the lifestyle of their choice. If you are a person 50 years or older who is homeless, or at risk of homelessness, the Assistance with Care and Housing Program can assist you to find the appropriate accommodation to meet your needs and choice. This may include assisting you to find housing in the private market, affordable social housing or entry to a residential aged care facility. Services also include tenancy issues, hoarding and squalor.
VincentCare Assistance with Care and Housing
The Assistance with Care and Housing Program supports those who are homeless or at risk of homelessness, to access appropriate and sustainable housing as well as community care and other support services, specifically targeted at avoiding homelessness or reducing the impact of homelessness.
VincentCare’s Assistance with Care and Housing Program provides services for older people in the Inner Gippsland Region comprising of the following local government areas:
• Baw Baw
• Latrobe
• Bass Coast

Assistance with Care and Housing supports vulnerable clients who are homeless or at risk of homelessness to remain in the community, through accessing appropriate, sustainable and affordable housing and linking them where appropriate, to community care and other support services.
