The WA Country Health Service (WACHS) is a State Government organisation providing comprehensive health services to country West Australians.
WACHS spans 2.55 million square kilometres and is the largest country health system in Australia covering the whole of the State outside the metropolitan area.
Despite vast distances, WACHS provides health services to residents and visitors that include emergency and hospital services, population, public and primary health care, mental health, drug and alcohol services, Aboriginal health, child and community health and residential and community aged care services.
WACHS provides aged and community care services for older people across our communities, including aged care assessments, community care, residential aged care, subacute and acute care, and transitioning older people from hospital back into the community.
WACHS is a provider of CHSP services such as domestic assistance, personal care, meals and food services, and transport.

Swan Hill Rural City Council (CHSP)
Domestic assistance helps maintain a safe, healthy home so frail older people and people with disabilities and their carers live independently at home and in their community. It provides vital housekeeping tasks that can’t be done by the client such as the bathroom, toilet, kitchen, living areas and bedroom or help with shopping.
An assessment will be undertaken to determine priority levels for all potential clients. A waiting list applies for this service.

• cleaning kitchens - sinks and bench tops
• making beds, some ironing, washing clothes
We welcome people with diverse needs, by treating you as an individual, respecting your needs and wishes. You will help our Coordinator to develop your own care plan for your service.

UMFC Support Options - Hospital to Home
Domestic Assistance: Short Term including Hospital to Home Support for people who are frail aged or have dementia. This could be for one off or intermittent support requirements or following discharge from a private hospital or a short term extension of post-acute care. If the person requires longer term support (beyond the 4 – 6 week period), this support would be via a longer term Commonwealth Home Support provider.
