Nursing care is provided by a Registered or Enrolled Nurse. Tasks such as recording client clinical observations may be conducted by trained and skilled staff, overseen and supervised by a Registered Nurse or Enrolled Nurse. Services are usually short term or episodic, depending on your assessed needs.
Nursing care can include:
• continence assessment, education and advice
• regular monitoring of blood glucose level, blood pressure etc
• education in self-management of chronic conditions
• short term wound care
• support with medication management.

We’re a team of locals who work with you to make sure you’re getting the services you need in the way you want to receive them. We cover the full range of nursing services, from medication help and wound care through to higher levels of support.
We provide care on a regular and consistent schedule throughout the week, ensuring that your care remains as consistent as possible.
Our nurses will monitor your health closely, ensuring you stay safe and informed. They’ll also coordinate with your GP to manage any health issues, helping you live comfortably and independently within your own home and community.
AccessCare Kingston, Home Support Program
Short-term or intermittent monitoring of medical conditions (e.g. diabetes, hypertension, lymphoedema, constipation, heart failure, cognitive impairment, dementia).
- Identifying causes of and implementing strategies to reduce carer stress
- Improving health literacy and supporting clients in communication and appointments with treating clinicians/GP’s.
- Assess for medication adherence issues and pill burden and refer as appropriate. (NOT medication administration)
- Falls risk assessments and implementation of falls prevention strategies.
- Improving Health literacy including around end of life planning including advanced care planning.
We are not the provider to refer to if client is requiring clinical care, e.g. wound care, medication administering, etc.
