We provide care on a regular and consistent schedule throughout the week, ensuring that your care remains as consistent as possible.
Our nurses will monitor your health closely, ensuring you stay safe and informed. They’ll also coordinate with your GP to manage any health issues, helping you live comfortably and independently within your own home and community.

BCH offer Nursing as part of in-clinic Cardiac Rehab. BCH do not offer community nursing. Cardiac Health is an 8-week reablement program that assists people with heart conditions to recover and get back to normal activities sooner. It also provides information to reduce the chance of a further event.
The program focuses on:
• learning more about your heart condition and treatment
• getting back to your usual activities (e.g. work, driving)
• changing your lifestyle to improve your heart health
• managing your medicines
• stopping smoking
• knowing warning signs and what to do in an emergency.
The program is for people who have had:
• a heart attack
• bypass or stent surgery
• stable or unstable angina
• controlled heart failure
• valve replacement surgery
• other vascular or heart disease.
The 8 week program includes 2 exercise and 1 education session per week. The program is lead by the Cardiac Nurse & Exercise Physiologists with dietitians and counsellors
