We can provide a range of nursing services in the comfort of your home such as medication management, continence and catheter care, chronic disease care for conditions like arthritis and diabetes, stoma care, and wound care.
Our expert nurses regularly review your progress and work with other health professionals like your doctor to ensure you receive the care and support you need.
Tamworth Community Health Service
Community based nursing provide assessment, monitoring, support and advice, health education, clinical nursing procedures, supervision, short term medication administration, wound care, and palliative care. Services are provided either in the home or a clinic setting. Community nurses work in partnership with other health professionals and community groups to provide culturally appropriate service provision to designated community groups.
By working in partnership with the hospital discharge planners, clients are assisted to maintain their independence and avoid prolonged hospitalisation by having clinical procedures attended in the community setting.
Home visits or Clinic appointments (at hospital or Coledale Centre) are offered.
