Community Nursing (District Nursing)
Community Nursing provides a seven day a week, 24 hour (including after hours support) home nursing service across the Geelong region. The service is staffed by Registered and Enrolled Nurses and facilitates client independence. The team provide nursing care in the home for people admitted to Hospital in the Home (HITH) and Post Acute Care (PAC)
Three Community Nursing teams are based at Belmont, Corio and Newcomb Community Health Centres and provide the following services:
Nursing care - following discharge from hospital
Palliative care -community nurses work closely with Community Palliative Care experts to support people in their home
Wound Care – community nurses provide assessment and management of leg ulcers, lacerations and skin tears or wounds that have not healed properly
Diabetes Management – community nurses support people to be independent in the community
Continence Care – support catheter care and changes.

Cognitive Assessment and Pathways Service - North West
The Dementia Support Service is provided by a Clinical Nurse Consultant who is highly skilled and is able to assess a person for dementia. The Service is designed to assist people who have difficulty with everyday tasks due to a disability. Services include clinical assessment to identify individual needs and goals; information about individual clinical management choices, support to reach individual management goals; education and information to individuals and groups.

We provide a full range of nursing services including District Nursing, Community Health Nursing, Diabetes Education, mens health groups, womens health groups and health screening.

Southern Plus : Bran Nue Dae Care Services
Southern Plus, offers a broad range of health and wellness services to support older people to remain independent, living at home for longer and to truly enjoy life as they age.
These services include:
• Domestic Assistance – cooking, ironing, cleaning, gardening, home maintenance
• Personal Care – showering, dressing
• Nursing – wound care, medication management
• Allied health – Physiotherapy, occupational therapy, podiatry
• Respite – flexible services, day and overnight respite available
• Transport – shopping, appointments, social activities
• Social Support – individual or group activities
