Latrobe Community Health Service - Dementia Nurse Practitioner
The Dementia Nurse Practitioner provides support to both clients and family members living with Dementia across Gippsland. They work in conjunction with the GP, and can provide both a primary and secondary assessment of the client with cognitive and or memory impairment, to assist with early intervention and diagnosis within the scope of the Gippsland Dementia Plan. They will work with the client to develop a care plan and refer to other services.
