The District Nursing Service conducts home visits to aged, frail and ill housebound residents to monitor health, assist with administering medication and provide support. Services include post-acute care, oncology support, wound management and palliative care.
A 24-hour emergency service is available for palliative care clients.

Resthaven Community Services - Manningham / Sefton Park
Nursing care is provided by a Registered or Enrolled Nurse. Tasks such as recording client clinical observations may be conducted by trained and skilled staff, overseen and supervised by a Registered Nurse or Enrolled Nurse. Services are usually short term or episodic, depending on your assessed needs.
Nursing care can include:
• continence assessment, education and advice
• regular monitoring of blood glucose level, blood pressure etc
• education in self-management of chronic conditions
• short term wound care
• support with medication management.

Byron Shire Community Nursing
Community Nursing Service provide and coordinate the delivery of accountable and responsible client centred care using an evidence based framework. Services include
• Acute and chronic wound management;
• Management of PICC lines and other venous access devices;
• Management of catheter and stoma care;
• Ongoing assessment, planning and evaluation of client care including making relevant and timely referrals to other services;
• Supporting Hospital with early discharge and avoidable hospitalisation.
Other clinical activities such as
• The provision of end of life care and support to palliative care clients and their families/carers including pain management and basic nursing support;
• The provision of chronic disease management activities such as, care coordination through the provision of self-management strategies, discharge planning from the inpatient services and ongoing case review.
