Wellness for Independence suite of programs builds on the latest research and focuses on enabling the successful prevention and management of chronic conditions in older people.Telehealth is the remote monitoring of a person’s vital health signs and symptoms in their own home. Providing clinical support to older people living in regional, rural and remote communities through the effective use of technology. It is intended to enable the older person to remain living in their own homes for longer by avoiding premature admission to residential aged care or unscheduled hospitalisation. Through better understanding and awareness of health conditions, and increased accessibility to health services driven by improved evidence based clinical decision making. Telehealth provides early intervention strategies, support self-management and promote healthy lifestyle choices among older people living in regional, rural and remote Australia
Central Bayside Community Health Services
Our caring and experienced nursing team provide a broad range of health and support including care planning, coordination of care and referrals. Our nurses help people with complex or chronic health problems to maximise their health potential and wellbeing.
Our Diabetes Education Service provides one-to-one support for people who are diabetic. This service is part of an integrated Allied Health team and coordinates care with Dietetics, Podiatry and other specialist services.
Nursing services are available at our centres, via Telehealth (phone or video consultation) or at clients’ homes

Services are provided to people living in our geographical areas. Community Health services focus on restoring, improving, or maintaining older people’s independent functioning and wellbeing also providing basic foot care at our foot clinic. The focus is to assist and support older people to either maintain or recover a level of independence, allowing them to remain living in the community. Some services are able to be provided in clients homes, others are provided at the provider’s location.
HNELHD Community Nurses work closely with the Allied Health Services in the Hunter New England Local Health District (HNELHD) will also work with you in partnership with Social Support Groups, also providing acute care, GPs and other service providers.
Services Provided: Dietetics, Occupational Therapy, Physiotherapy, Podiatry, Social Worker, Mental Health and Speech Therapy.
