Community Nursing Service provide and coordinate the delivery of accountable and responsible client centred care using an evidence based framework. Clinical services provided include;
• Acute and chronic wound management,
• Management of PICC lines and other venous access devices,
• Management of catheter and stoma care,
• Ongoing assessment, planning and evaluation of client care including relevant and timely referrals to other services,
• Supporting Hospital with early discharge and avoidable hospitalisation.
Other clinical activities provided include;
• The provision of end of life care and support to palliative care clients and their families/carers including pain management and basic nursing support,
• Hospital in the Home,
• The provision of chronic disease management activities such as, care coordination through the provision of self-management strategies, discharge planning from the inpatient services and ongoing case review.
Goulburn Valley Health Home Nursing Service: District Nursing
Goulburn Valley Health District Nursing Service provides specialist home nursing services including assessment of broader care needs to a culturally diverse community within the City of Greater Shepparton area. It is responsible for taking an active service model approach in improving the health status and independence of the consumer and supporting the carer in their caring role. Goulburn Valley Health District Nursing Service helps frail, older people and younger people with disabilities and their carers living in the community. The service's role is to maximise clients' independence through the delivery of timely, high quality entry-level support services, taking into account each person’s individual goals, preferences and choices – and underpinned by a strong emphasis on wellness and reablement.
