Country Health LHN - Transition Care Programme (TCP)
Transition Care Programme (TCP) provides a range of short-term care services to eligible older people after they leave hospital. TCP services include low intensity therapy (e.g. physiotherapy, occupational therapy), nursing support and/or personal care that allows older people to continue their recovery out of hospital while appropriate long term care is arranged. To enter TCP a person must be assessed as eligible and approved for care by an Aged Care Assessment Team (ACAT) while in the hospital, and must enter the programme directly upon transfer from hospital. TCP can be delivered in either a residential facility setting or in a community setting (e.g. the person’s own home). Where the service is delivered is dependent upon the type of care needed.
