Our expert multidisciplinary care teams have helped many Australians improve their mobility and independence, all from the comfort of their own homes.
Our dietitians can provide advice and assistance to eat and feel well, including improving appetite and managing dietary requirements.
Physiotherapy services can help you to maximise your strength, balance, and mobility through targeted exercises and support. Our occupational therapists can help you live safely at home with home modifications, mobility equipment, and pressure management techniques.
Podiatry services can help you move freely and feel comfortable on your feet. We can provide footwear assessments and treat foot conditions. Our speech pathologists can support you to eat, drink, and communicate by addressing swallowing problems, improving speech, and assistive technology support.
We also have social workers to help you improve your independence, manage relationships, financial matters, and unsafe housing.
Social Work – Brookvale
Social workers engage in interpersonal practices including psychosocial assessment of clients and their carers, case work and counselling for those clients and carers who are facing a crisis, illness or complex social situation. Social workers aim to enable and empower clients and their carers with the support and resources they require to remain in their home or to consider alternative options for long term care and accommodation. This may be achieved by referrals and advocacy to support services e.g. ACAT, respite services and other community support services. Social workers provide intervention regarding complex matters such as elder abuse or Guardianship issues.

Eastern Health Community Health (Healesville, Yarra Junction and Angliss Hospital)
Our aim is to improve the physical, social and emotional wellbeing of our community.
We cater for all ages and also offer specialised services for people with chronic conditions. We provide individual sessions and group therapy services such as exercise classes and groups. Our clinics are located in Healesville, Yarra Junction and Angliss Hospital and we also offer home-visiting and outreach services.
Our fees are charged based on your income, starting from just $10 per consultation. Counselling and Nursing (Tobacco Free Clinic) services are free. Fees can be waived for people experiencing financial hardship.
