At Uniting AgeWell (UA) we believe older people, like people of all ages, want to live in an environment of choice, empowerment and wellness, able to access support and care as they choose. UA Southern Tasmania offers a wide range of services to enable older people to remain active and independent, and living in their own communities, including Home Care Packages, Centre-based Respite for Home and Community and National Respite for Carers.
Our highly skilled team works closely with individuals and groups to help them achieve their personal goals.
Our Home Care Package service covers Level 2 and Level 4 clients. Services and programs include referrals to physiotherapy, occupational therapy, falls prevention, health promotion education and social activities.

Vision Australia’s occupational therapists provide advice, recommendations, demonstration or training on strategies on grooming, maintaining health and fitness, clothing care and management, management of medication, identification of money, telephoning skills; preparing meals and safety etc.
Podiatry - Hornsby
SERVICES PROVIDED AT HORNSBY HOSPITAL AND 6 COMMUNITY HEALTH CENTRES IN OUR CATCHMENT AREA. CLIENTS MUST LIVE IN HORNSBY OR KU-RING-GAI LOCAL GOVERNMENT AREAS.
Podiatrists treat conditions of the feet and provide assessment, treatment and education to assist in managing and preventing complications including ulceration, amputation and infection in high risk clients. The services aim to preserve mobility and independence and prevent hospitalisation.
Referrals are accepted for people who have specific high risk medical conditions including diabetes, renal disease, rheumatoid arthritis, poor circulation and numbness of the feet. Interpreters are available. A medical referral is required.
Personal care such as toenail cutting and management of calluses for people without chronic medical conditions is unable to be provided.
Clients who are unable to attend hospitals or community health centres can be referred to the Lane Cove and North Side Community Services to access In-Home P
