Based on an individual assessment, allied health professionals work with you to establish a tailored treatment plan that can include individual or group therapy. Services may be provided on a short term or episodic basis and are provided to support your independence with a preventative, maintenance, or restorative focus. Services may include:
• Podiatry
• Occupational therapy
• Physiotherapy
• Formal counselling
• Speech pathology
• Nutritional advice
• Assessment for home safety and equipment needs
• Education in use of equipment that supports independence
• Exercise/Gym classes/Falls prevention
• Tai Chi, Chi Ball
• Condition-specific (Parkinson’s, stroke/pulmonary conditions, arthritis, diabetes) self-management.
Sessions are conducted at Paradise (Aquatic exercise venue is in Modbury) with some services provided in your home (eg Occupational Therapy). Transport to and from venue may be provided, depending on your circumstances.
Uniting Communities Health Access
Uniting Communities' Health Access offers Allied Health Services promoting wellness and reablement to support you to maintain your physical, cognitive and social health and wellbeing to enable you to continue living at home, in your community as independent and active as possible. We offer a range of services: Physiotherapy, Social Work, Occupational Therapy, Home Safety Assessments, Podiatry, Hydrotherapy, Arthritic Hand Group. Cognition groups including Brain Gym and Active Brain plus many exercise groups: Falls/Balance Class, Strength Class, Pilates, Easy Movement Active Ageing and Chi Ball.
