Whether your goals are for better physical health, improved safety and functional independence, greater emotional resilience or an enhanced sense of wellbeing, our experienced team of Allied Health professionals will work with you to develop a plan to achieve your desired outcomes. You may benefit from individual therapy such as
• Physiotherapy – for pain relief and management strategies, reconditioning and therapy for specific conditions e.g. stroke, MS and other neurological disorders
• Occupational Therapy – for advice about home safety and prescription of home modifications, equipment needs and issues of cognitive decline
• Speech pathology – speech and swallowing disorders
• Podiatry
We also provide a range of group programs supervised by the allied health professionals.

cohealth’s health services can help you maintain your life at home. We can provide better care when we can work with you to coordinate a range of services to support your recovery and wellness.
We tailor and coordinate a Care Plan with you. It is designed to meet the needs identified through your My Aged Care assessment. Your Care Plan will include a range of cohealth services or other services in the community.
Remember to ask for cohealth services when you contact My Aged Care. We are here when you are ready to start planning your care.

Melton Community Health Centre - Djerriwarrh Health Services
Djerriwarrh Health Service offers nursing and allied health services for individuals and community groups. We provide a multidisciplinary approach to care. Services are provided at our centre or in your home if required.
We offer Physiotherapy, Exercise Physiology, Nutrition and Dietetics, Podiatry, Occupational Therapy (OT), Speech Pathology, Counselling, Group Exercise Programs, Hydrotherapy, Community Nursing, Wound Care, Health Coaching, Diabetes Education, Continence.

BASScare Social Support Services
Social Support Services provide a range of services for older people and for older people living with a disability in Boroondara and surrounds. These services are designed to enhance health and wellbeing and to promote friendships and strengthen community participation. All the groups are run by qualified staff and are supported by volunteers from our local community six days a week from our centres in Canterbury and North Balwyn. Planned activity groups include; community dining room, exercises classes, art and craft, hairdressing and podiatry.

The WA Country Health Service (WACHS) is a State Government organisation providing comprehensive health services to country West Australians.
WACHS spans 2.55 million square kilometres and is the largest country health system in Australia covering the whole of the State outside the metropolitan area.
Despite vast distances, WACHS provides health services to residents and visitors that include emergency and hospital services, population, public and primary health care, mental health, drug and alcohol services, Aboriginal health, child and community health and residential and community aged care services.
WACHS provides aged and community care services for older people across our communities, including aged care assessments, community care, residential aged care, subacute and acute care, and transitioning older people from hospital back into the community.
WACHS is a provider of CHSP services such as domestic assistance, personal care, meals and food services, and transport.
