Hutt Street Centre provides access to Occupational Therapy services for both on-going and episodic support to people who are at risk of homelessness. The Occupational Therapist aims to assist the older client to maximise their strengths and build skills to participate effectively in everyday activities. Occupations are seen as the everyday activities that occupy time and energy and give meaning to people’s lives. The Occupational Therapist completes practical standardised and non-standardised assessments and interventions that support the older client’s needs. These can include: Functional and cognitive assessments; Home safety assessments; Motivational interviewing and intervention; Sensory screening and intervention. Occupational Therapy services are provided individually and in group settings, both in the home and on site.

Our Community Care Podiatry Service team offers a variety of short-term and long-term podiatry care to residents of the ACT who hold a Centrelink Pension Card or Low-income Health Care Card.
Our services include:
• General podiatry treatment
• Foot health assessment
• Biomechanical assessment
• Footwear and orthotic prescription
• Nail surgery
• Health promotion and education.

Hours of operation: 8.30 – 5.00 Monday to Friday (excluding Public Holidays)
No home visiting available

Who can attend:
• Initial assessment and advice only available for ACT residents holding a Centrelink Pension or Health Care Card

Home Nursing Group provides community aged care services to support you in the home you love, surrounded by family and friends and connected with your community. Home Nursing Group can provide access to Allied Health and Therapy Service delivered by qualified professionals at the their consulting rooms.

PLEASE INCLUDE REFERRAL CODE IF AT-HM WILL BE REQUIRED. REFERRALS FOR GAIT AID ASSESSMENTS SHOULD BE REFERRED TO PHYSIOTHERAPY
There is currently a 6+ month wait for OT services. Please speak to clients about the wait prior to making a referral.

We tailor and coordinate a Care Plan with you. It is designed to meet the needs identified through your My Aged Care assessment. Your Care Plan will include a range of cohealth services or other services in the community.
