Resthaven Community Services - Marion
Allied health and therapy services: based on an individualised assessment, qualified and registered allied health professionals work with the client to establish a tailored plan of treatment that may include individual or group therapy, exercises, self-management of chronic conditions. Services are provided by or under the supervision of allied health professionals, with a preventative, maintenance, or restorative focus, to support the client to be independent.
Services include:

• Nutritional advice from a qualified dietician
• Education in use of equipment that supports independence
• Exercise/gym classes
• Walking/falls prevention/balance groups

Condition-specific (Parkinson’s, stroke/pulmonary conditions, arthritis, diabetes) group sessions are aimed at peer support, self-management, education and supporting wellbeing.

Footprints Community
At Footprints our aim is to support people, including those with complex needs and challenging behaviours, to remain independent and living the lifestyle of their choice. Using a person centred approach, Occupational Therapy services at Footprints support individuals to improve their quality of life, prevent functional decline and maintain engagement in meaningful activities, Interventions include prescription of mobility and daily living equipment; falls risk assessments and falls prevention strategies and functional assessments of daily living activities. Assessments undertaken by Footprints’ Occupational Therapist include review of clients’ home environments to identify any modifications required to meet functional need to promote wellbeing and reablement and prevention of functional decline. Information is also available on strategies to improve function and enable participation in daily living, leisure and social activities.
Community Health - Noosa
Community Health Occupational Therapists prescribe equipment and home modifications to improve the level of function of individuals who have a condition or disability which impacts upon independence, including conditions associated with ageing. Occupational Therapists visit individuals in their home and provide intervention for falls prevention, pressure injury management, and increasing the safety associated with activities of daily living such as showering, toileting and transfers. They complete home safety assessments and applications to the Medical Aid Subsidy Scheme for equipment where appropriate.
