1. The referral must specify “Occupational Therapy” as the service sub-category.
2. Referrer must only complete one referral, completing multiple referrals to occupational therapy does not increase the speed at which a client is processed.
3. Reason for referral must be included in “referral comments”. Link Health & Community service coordination team will not be viewing support plans/comprehensive assessments to gather more information about a referral. If the criteria for referral is not met in “referral comments” the referral will be rejected requesting “further information to be added”.

Exercise Physiologists at Link Health & Community can educate, assess, advise and prescribe exercise interventions to: help manage and prevent chronic conditions; improve strength, fitness, balance and mobility to enable you to do the things you love more easily, and correct underlying causes of pain and injury to stop it reoccurring.
